    2. Is your current mailing address the same as the above property address?*
    3. Reason for different mailing address*
    4. From which account that has a credit balance are you requesting a refund?
    5. Did you, as the current or former registered owner, make the payment(s) which caused the credit balance in the account(s)*
    6. Input N/A if the payer was your mortgage company.

    7. This file should display your name, City of Coquitlam as the payee, the dollar amount and date of the payment made in error, and the account number that the payment was made to.
