Applicants are encouraged to create an account with their online application. Creating an account enables you to save your application and complete it over multiple sessions. Provide the full legal name of the organization.
If the organization normally uses something different from the legal name, provide that here.
Select 'yes' if the organization has a base of operations within the City limits of Coquitlam.
Select 'yes' if the organization does not owe money to the City of Coquitlam.
Name the area in which the organization works. For example: Tri-Cities, Lower Mainland, Canada, Burke Mountain, etc.
Provide the Society's registration number.
Provide the year the Society was registered.
(Include details about the structure of the board of directors, management and staff where applicable.)
Describe how the organization is structured: How are board members selected? How many staff? How many volunteers?
Indicate how the organization became aware of the Spirit of Coquitlam Grant.
Provide the name and information for the contact person who is authorized to speak on behalf of the organization with respect to the application.
If possible, please provide the name and contact information for an additional contact.
Provide the organization's mailing address for all correspondence regarding the grant application.
Provide the organization's physical address if it is different from the mailing address.
Include the website address, social media handles, and/or other ways that the organization connect and communicates electronically with the community it serves.
The City keeps a database of not-for-profit organizations that are interested in receiving information that may be of use to them.

Write the name that you would like the City to use for the project. If successful, the project title will be shared on the City's website.
Include the total cost of the project. This amount should be the same as the amount on the budget template submitted as part of this application.
Include the total amount of money being requested through the Spirit of Coquitlam Grant. This amount should be the same as the amount on the budget template submitted as part of this application.
Tell us when your project will begin and when it will end. If you are applying under the 'Festivals and Events' or 'Sport Hosting' stream, include the date(s) of your event as well.
Note: Project start and end dates are not usually the same as the event date.
Indicate where your project will be delivered. Describe it in a way that makes most sense to you - will it be at a park? In a specific neighbourhood?

Try to be as specific as you can about who will be able to participate or benefit. General statements like 'all youth in Coquitlam can access this service' do not help the committee to understand the value of the project. Include participant numbers, age, gender, etc. as appropriate.

Estimate how many people in question 3f come from each of the areas lists.
Provide a brief description of the project, 150 to 300 words max. The City may use the summary in part or in full to report on successful grant recipients.
Describe why your organization exists and how this funding will help the organization meet its goals.
Describe who your organization serves. This is where it may be useful to include some more general statements like 'youth in the Tri-Cities'.
Check the box that best represents where you expect people who participate in your projects to be located.
Tell us how the project will benefit the community it serves and the broader Coquitlam community.
Select the strategic goal you feel your project best represents.
Talk about how your organization determined that this project would be good for the community.
Is this project filling a programming gap? Are there barriers to people participating in an activity or process? Is there a need for more knowledge on a particular topic?
Please list the different survey and feedback methods you plan to use and the type of information you will collect (e.g., number of participants, age group of participants, number of new members, participant satisfaction feedback, before and after surveys).
If you receive a Spirit of Coquitlam Grant, you will be asked to reflect on the success of your project in the final report.

If your project is intended to be open to all members of the community, tell us how your organization will actively include people with diverse experiences.
If your project is focused on serving a particular group, tell us about who is included in that group.
If relevant, talk about steps you are taking to make your project environmentally sustainable.

Tell us how much you will charge people to participate in your project.
Include details about different charges for different groups, any subsidies that may be available, and any other information related to how you will determine the cost to participate.
If relevant, tell us about the involvement of volunteers in your project.
Include details such as the number of volunteers, the total number of anticipated volunteer hours, their tasks, how you will recruit volunteers, etc.
List confirmed and expected funding. If you have applied for a BC Community Gaming Grant, please include information about that application including the amount and the intended use of the funds. If available, please list the approximate dates for receiving the results of expected funding.

The undersigned agree to the conditions of the Spirit of Coquitlam Grant, as outlined in section 6 of this application, and confirm that the individual noted in section 2 is authorized to be the City’s liaison with respect to this grant application.

In the event of a Freedom of Information (FOI) request regarding the present funding application or any other information about the applicant in the City's possession, the information provided to the City will be treated in accordance with the Freedom of Information and Protection of Privacy Act (FIPPA). Where funding is approved, however, the amount of funding, the purpose for which the funds were granted and the name of the organization receiving the funding are considered public information.
The personal information collected on this form is collected in accordance with the Freedom of Information and Protection of Privacy Act. The City has authority to collect your information for the purposes of administering the Spirit of Coquitlam Grant.
