    1. The City of Coquitlam strives to provide accessible recreation opportunities.  After reviewing this form, a Community Services Coordinator will be in contact with you to discuss the recreation support plan.  

      Recreation Support Plans for successful participation

      A recreation support plan can contribute to the participant’s experience by: 

      • Providing key information to program staff on how best to support the individuals unique needs
      • Identifying strategies to help the individual stay engaged and have fun
      • Having supports such as visual cards and noise minimizing headphones if needed 
      • Ensuring program spaces meet accessibility requirements 
      • Providing one to one inclusion support for participation when available (see below for more information)
      • Caregivers can arrange for their own support person (EA, BI), trusted family member or friend (over 16 years) to accompany the participant if needed. 
      • If participants require extra support and/or strategies for successful participation in recreation programs and camps, a RECREATION SUPPORT PLAN needs to be completed a minimum of one month prior to the start of the program each season.

      One to One Inclusion Support in Camps 

      The community services department will work with families to provide one to one support for participants who require it for out of school camps. It is recommended that if the participant has a support person at school that you apply for one to one support for camps.

      Our ability to accommodate may depend on:

      • Level of support requested and available resources at the time of registration
      • Behaviours beyond the scope of a recreation support plan may include: physical and verbal aggression, wandering/running from the group, other behaviours placing staff and/or participants in unsafe situations
      • Recreation staff do not administer medication or directly assist with feeding, dressing or toileting
      • Ensuring program spaces meet accessibility requirements

      One to one support is provided by City of Coquitlam Inclusion Support Leaders (ISL’s). Our ISL’s have education and experience in one or more of the following areas:  

      • Education Assistant, 
      • Therapeutic Recreation Assistant, 
      • Behavioural Intervention

      Any registrations not listed on this form require completion of a new request form.

      Participants who require direct support to safely enjoy our services can also bring their own support person with them at no additional cost. It is expected the support person will be with the participant throughout their visit and be fully engaged in supporting them. Please let the front desk or programming team know upon arrival.

      All support is subject to availability of staffing resources and demand. The City does their best to provide resources to as many participants who need it as they can.
